For Baseball Camps, Clinics, and Lessons at Windermere Preparatory School
Acknowledgment of Risk and Assumption of Responsibility
I First Name*Last Name* understand and acknowledge that participation in baseball activities, including but not limited to throwing, hitting, fielding, running, strength and agility training, and other athletic exercises, involves inherent risks. These risks include, but are not limited to, bodily injury, concussions, sprains, fractures, heat-related illness, and, in rare instances, permanent disability or death.
I affirm that I (or my child) am voluntarily participating in the camps, clinics, and/or lessons offered by Oturns2 LLC at Windermere Preparatory School, and I assume full responsibility for any and all risks, known or unknown, associated with such participation.
Release of Liability and Indemnification
In consideration of being allowed to participate, I hereby release, waive, discharge, and covenant not to sue:
- Oturns2 LLC
- Omar Peña
- Windermere Preparatory School
- and any and all employees, volunteers, contractors, officers, directors, partners, affiliates, and agents,
from any and all liability, claims, demands, causes of action, damages, costs, or expenses, including legal fees, arising from or related to any injury, illness, property damage, or death that may occur during participation in the program, regardless of whether caused by the negligence of the above parties.

I further agree to indemnify and hold harmless all the above parties from any claims, damages, or expenses brought by a third party arising from my (or my child’s) participation.

Medical Authorization
I authorize Oturns2 LLC and its representatives to seek emergency medical treatment in the event of an injury or illness. I understand I am financially responsible for any medical services rendered.

Photography and Media Release
I grant permission for Oturns2 LLC to take photographs and videos of me (or my child) during participation in the events for promotional use on websites, social media, and marketing materials. I understand that no compensation will be provided.

Acknowledgment and Understanding
I acknowledge that I have carefully read and fully understand the terms of this waiver and release. I understand that by signing this document, I am waiving certain legal rights, including the right to sue. I certify that I am at least 18 years of age or that I am the parent or legal guardian of the minor participant listed above.
