I understand that this booking request form does NOT guarantee me an appointment. I will be contacted for follow up within 3-5 business days, and be required to send a non-refundable deposit fee to confirm my appointment. I acknowledge that until the deposit has been received, my appointment has not been confirmed. Should I need to cancel or reschedule the appointment, I will cancel/reschedule no later than 48 hours before appointment day to avoid losing my deposit.
