Emergency Medical Form

Purpose: To enable parents and guardians to authorize the provisions of emergency treatment for children who become ill or injured while under Sophistication Dance Company’s authority, when parents or guardians cannot be reached.
Part 1- To Grant Consent

In the event reasonable attempts to contact me (parents/legal guardian) have not been successful, I hereby give my consent for treatment deemed necessary of any preferred physician or preferred dentist (or in the event the designated practitioner is not available, by another licensed physician or dentist). If preferred or licensed physician/dentist is not available, I give my consent to transfer my child to my preferred hospital or any hospital reasonably accessible.
This authorization does not cover major surgery unless the medical opinions of two other licensed physicians or dentists, concurring in the necessity for such surgery are obtained prior to the performance of such surgery.

PRACTICE PICK UP/DROP OFF

Practices are CLOSED. Dancers are to be dropped off at the front entrance door of the studio no sooner than 5 minutes before their class time. All dancers must be picked up promptly. If your child does not have practice or it is not their practice time, they are not to be at the studio without permission from the SDC Staff. There is a CHILD CARE FEE of $15.00 for every 10 minutes you are late that you must pay the coach that has to stay after practice to wait for you to return to pick up your dancer.
ATTENDANCE POLICY

Attendance is a huge part of dance. Missing class can make our job as choreographers/coaches difficult and negatively impact your dancer’s team. If your dancer has to miss practice for any reason, this must be communicated to your coaches PRIOR to practice starting. Being in attendance to classes is strongly encouraged for all dancers. ***Practice the week leading up to a recital or competition is MANDATORY for all dancers! If your dancer misses their group practice the week leading up to a recital or competition, they may not be permitted to perform in that recital/competition. Only excused absences will be permitted before a recital/competition. Excused absences include: Documented illness (with Doctor’s note), mandatory school function, or death in family.
COMPETITION/RECITAL ETIQUETTE

For recitals, all dancers must arrive dressed and ready at their assigned time. All dancers must stay for the entire recital. For competitions, times will be sent to parents for when dancers need to be ready and in the room, which need to be followed at each competition. If you choose to get your dancer ready before you arrive at the school, you are still required to be there at the assigned time. If you leave the competition venue, you must notify your coach that you are doing so and you must return in time for the dancer to be ready at the assigned time for their next performance. All dancers must stay for awards, unless excused by Rilee.
CONTACTING COACHES

The use of Sophistication Dance Company’s name or logo may not be used for solicitation purposes without the consent from the director. ***Under NO circumstances will any negative comments discrediting our company or the SDC staff be tolerated. This will be grounds for your dancer(s) immediate dismissal from the company. If you have any questions or concerns, you may speak with any of the SDC staff BEFORE or AFTER practices. You may text or call coaches before 9:00pm as well. We ask that you do not discuss issues with coaches during practices, recitals, or competitions, as these are never the time or place to address concerns. All decisions made by the SDC staff are made with the hopes of building a strong and positive company. We always have our dancers’ best interest at heart when making decisions. We are a dance family and expect everyone to show RESPECT to everyone.
TAKING OUTSIDE CLASSES

SDC dancers are not permitted to take outside dance classes at other local facilities/studios unless prior approved by SDC staff. (This does not include college camps/intensives).
2026-2027 Fees
Payments are due on the 1st of every month. There will be no refunds of any kind. There will be a $35.00 late fee applied to your account on the 6th day of the month for ANY balances that are still due on your account. If your account is late for 2 consecutive months, your child may be asked to leave the company. If your account is not current, we will not order your child’s costume and your child will not be able to compete and/or perform at any recitals. You may call or text Rilee for questions you have. You may only discuss concerns regarding your account with Rilee. If you choose to leave the competitive dance team after the season has started (August 1st) and before the end of the season (final recital) you will be charged a $250.00 fee. SIGNING THIS FORM AND CHOOSING OUR COMPETITION TEAM, YOU HAVE AGREED TO THIS FEE AND ALL FEES DISCUSSED!
REGISTRATION FEE

$50 due at the time of registration
COMPETITIVE DANCERS

$100 per month (includes group practice and technique class). $115 a month (includes group class, technique and tumbling class). $100 costume fee PER costume.
COMPETITION FEE FOR ADDITIONAL SIBLINGS

$90.00 per month (includes group practice, technique, and tumbling class)
COMPETITION FEES

Competitions are NOT included in the monthly fees! These are typically $60-$80 PER DANCER PER ROUTINE!! These will be paid for by Venmo, cash, or check. Competition schedule will come out by August/September. Some competition fees are not set per dancer, (meaning each team has a set fee regardless of how many dancers there are) these will be divided by how many girls are on the team. Payments for competitions will begin in September on dates determined by staff.
TEENY TINYS (ages 2-4, 2 recitals)

$55 per month. $80 one-time costume fee (Due October 1st). Possible additional costume fee for Spring recital.
TINYS (ages 5+) *Intro to Competitive dance! (1-2 competitions, 2 recitals)

$65 per month (includes 1 group practice a week). $80 per month (includes 2 practices a week, group and tumbling class). OR $90 a month (includes 3 practices a week: group practice, tumbling, and technique class). $100 one-time costume fee. $60-80 competition fee PER competition. ***TECHNIQUE class is only available for ages 7+
RECREATIONAL TEAM (non-competitive, 2 recitals) Ages 5+

$65.00 per month (includes 1 group practice a week). $80 per month (includes 2 practices a week: group practice and tumbling class). OR $90 per month (includes 3 practices a week: group practice, tumbling, and technique class). $100 one-time costume fee. ***TECHNIQUE class is only available for ages 7+
TUMBLING/TECHNIQUE CLASS ONLY

$65 per month
I understand that dancing/tumbling can involve the risks of serious injury. These risks of injury include the possibility of neck and spinal injuries, and other injuries to the body, nerves, joints, ligaments, muscles, etc. I also understand that permitting my child to participate with Sophistication Dance Company is to subject my child to the possibility of injury as stated above. In consideration of Sophistication Dance Company, permitting my child to participate in dancing/tumbling programs, I hereby agree to assume all risks of injury to my child associated with this company. I release, discharge, and agree to hold harmless Sophistication Dance Company, and their staff from any liability from any and all claims, which is any manner, arises from or as a direct or indirect result of this service.

In addition, I give my consent to Sophistication Dance Company to use/publish my child's photograph for studio advertisements, promotions and/or media release. By signing the assumptions of risks and release, I acknowledge that I have read and understand its contents and warning, and that I agree to this form.
