I/We understand that Happy Cats Rescue Project reserves the right to obtain background information on all applicants that is available via public records for the purposes of this application.

All of the above information I have given is true and complete. If we are chosen to adopt from Happy Cats Rescue Project , the animal will reside in the home as a pet.

I will provide him/her with adequate water, a high quality food, shelter, training, affection and medical care.

I understand it is my responsibility to evaluate the animal for myself before agreeing to adopt him/her. I am in full agreement with the terms of the adoption.

Happy Cats Rescue Project shall not be liable for, and is hereby relieved from, all liability for any damage, expenses, causes of action, fines, suits, demands, judgments, or claims of any nature whatsoever arising from or by any reason of any damage to property or injury to any persons caused in whole or in part during the visitation of the animal up for adoption and placed in your home.

I hereby accept and assume such liability and agree to protect, indemnify and hold the Happy Cats Rescue Project harmless from and against all of the aforesaid.

Completing, signing and returning this document constitutes your permission for Happy Cats Rescue Project to verify all information provided that you have provided and are attesting to as being true and correct.
