    Unsafe interaction with scaffold

    We’ve had instances where people have adjusted or moved scaffold components without authorisation. This creates serious fall and collapse risks.


    Angle grinder incident

    A recent incident occurred due to possible incorrect handling of an angle grinder. No major injury this time, but it highlights a gap in safe‑use knowledge.
     
    2. Key Safety Reminders


    Scaffold Safety

    No one is to touch, alter, move, or adjust scaffold unless they are a licensed scaffolder or have explicit approval from the site supervisor.
    Report anything unsafe immediately — loose planks, missing ties, damaged components, or unauthorised access.
    Never climb outside designated access points such as ladders or stairs.
    Keep loads within rated capacity and avoid storing excess materials on platforms.


    Power Tools – Angle Grinder Focus

    Only use tools you are trained and competent to operate.
    Inspect before use: guard in place, disc appropriate for the task, no cracks or damage, power lead intact.
    Use correct PPE: face shield, safety glasses, gloves, hearing protection, long sleeves.
    Maintain stable footing and secure the workpiece before cutting or grinding.
    Never remove guards or modify the tool.
    Stop work immediately if the tool vibrates excessively, overheats, or behaves abnormally.

  • 3. Improvements Going Forward


    Improvement 1 – Scaffold Control
    A “No unauthorised adjustments” rule is now in effect.
    Any scaffold concerns must be reported to the site supervisor or the scaffolding contractor.
    Daily visual checks will be added to the morning routine.


    Improvement 2 – Power Tool Competency
    A short refresher session will be held this week on safe use of angle grinders and other high‑risk tools.
    Workers must confirm they understand correct operation before using these tools onsite.
    Damaged or questionable tools must be tagged out immediately.


    Improvement 3 – Incident Reporting
    All near misses, even minor ones, must be reported so we can prevent repeat events.
    Quick reporting helps us fix hazards before they injure someone.
