Welcome to Summer Art Camp 2025! We are so glad you decided to join us. Please fill out the following paperwork completely and submit before your child's first day of camp. Parents/guardians must also read and review the parent handbook (linked below) before signature and final submission.

If you have more than one child, you will need to complete this form for each separately.

If your child is signed up for more than one week of camp or returns for an additional week of camp after you have completed this paperwork, you do not need to fill out these forms out again.

I, the above signed parent/guardian:
- Agree to assume all risks incidental to participation in art camp.
- Agree that the Armory Art Center, its owners, officers, employees and instructors are not in any capacity personally responsible or liable for any injuries or damage resulting from my child's participation in any Armory Art Center camp programs.
- Am aware that injuries may occur during my child's participation in art camp and understand that the Armory Art Center and its instructors take every precaution to prevent injury.
- Am aware that there is no physician or nurse present during camp to assist in an emergency situation.
- Verify that my child is in good health and has no other medical issues other than what is shared on the contact form.
- Understand that the Armory Art Center will not administer, dispense or hold any medications. Parents must make other arrangements if their child needs to take any medications.
- Give the Armory Art Center permission to acquire emergency medical treatment if it deems necessary.
The terms of this agreement may not be modified by any employee, representative, agent and/or assigns of the aforementioned parties. Participant agrees to be solely liable and save harmless the Armory Art Center, its perspective employees, assigns and/or agents and representatives from liabilities, losses, injuries, damages or claims or any kind or nature arising out of his/her child's' participation in camp activities of any kind or nature. Participant hereby waives any and all rights of recovery arising as a matter of law or otherwise which you might now or hereafter have against the Armory Art Center, its respective employees, assigns and/or agents and representatives as set forth above.

Photography Agreement

The Armory has the option to photograph your child unless the camp director is notified otherwise. This includes photographs, videos, and other records of classes or the programs involving the child used for any legitimate Armory purposes including social media.

Summer Camp Handbook

The Summer Art Camp Handbook can be accessed here. The handbook information must be reviewed before signing and submitting this document.
