The food safety pack must be provided for each confirmed catering operation and the event organisers will forward a copy of this form to Environmental Health Officers responsible for food safety.
There are 3 sections to the form:
- Food Safety Questionnaire
- Gas safety checklist
- Food Safety Management Declaration.
Any evidence you cannot include in the completed form you provide, must be uploaded to this form you are currently completing. Any missing or out of date documentation must be provided before your arrival at the event.
You must upload a completed copy of the document containing all 3 completed sections as part of this application.
