By signing below, I acknowledge and agree to the following:
I understand that participation in martial arts involves inherent risks, including but not limited to physical injury, illness, or even death. I voluntarily assume all risks associated with participation.
I hereby release, waive, discharge, and covenant not to sue Ignite Taekwondo, its owners, instructors, employees, agents, and affiliates (City of Lauderhill) from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained while participating in any martial arts activity, whether caused by negligence or otherwise.
I certify that the participant is in good physical condition and has no conditions that would prevent safe participation. In case of emergency, I authorize the provision of necessary medical treatment and accept responsibility for any related costs.
This release shall be binding upon me, my heirs, executors, administrators, and assigns. If the participant is a minor, I certify that I am their parent or legal guardian and have the authority to sign this waiver on their behalf.
