I, the undersigned applicant, hereby authorize H3 Cleaning Solutions and its designated agents and representatives to conduct a background check as part of my application for employment, promotion, reassignment, or retention.
I understand that this background check may include, but is not limited to:
- Verification of personal information
- Criminal history records check (local, state, and federal)
- Sex offender registry check
- Employment history and references
- Education verification
- Motor vehicle records (if applicable to the position)
I understand that the information obtained during this background check will be used solely for employment purposes and will be kept confidential, in compliance with the Fair Credit Reporting Act (FCRA), the Equal Employment Opportunity Commission (EEOC) guidelines, North Carolina state laws, and any other applicable federal or local regulations.
I acknowledge that I have the right, upon written request, to obtain a copy of any background check report and to dispute any inaccurate or incomplete information with the background check agency.
This authorization shall remain valid throughout the course of my employment with H3 Cleaning Solutions, provided such checks are related to my employment or position, and are permitted by law.
I release H3 Cleaning Solutions, its employees, agents, representatives, and any individuals or entities providing information from any and all liability for the receipt and use of such information in connection with my application and potential employment.
