  • CCFR Policy is that we will not adopt to or allow fostering in a home with any unaltered animals (cats or dogs) and we require pets in the home to be utd on core vaccinations. The only exception is if a pet cannot be altered or vaccinated due to detrimental medical issues accompanied by a vet note. 
  • Will the current pets in the home willingly accept a new dog/puppy into the home?*

  • Some dogs have problems with separation anxiety, fearfulness, or dominant behavior. Please select those characteristics you would be willing to help retrain either with Rescue guidance or with the instruction of a professional dog trainer or behaviorist.
  • If necessary, would you be willing to take a foster to basic obedience training (CCFR’s expense with pre-approval)?*

  • I understand that CCFR will be responsible for any medical, training, or other approved expenses associated with the foster dog in my care. I understand that by signing this form, I agree to release and hold harmless CCFR and its members from any claims, damages, costs, or actions including any financial costs from bites or injury to family or visitors or injury to other animals or property damages incurred as a result of the foster care or actions of the foster dog

    I understand I am bringing in a dog that CCFR does not always know the history on and I will use extreme caution and exercise good judgment when exposing the foster dog to my own animals, neighbor animals, family members and visitors since it is possible that a foster dog, until fully assessed, may demonstrate behaviors that need to be monitored. 

    I will notify CCFR immediately should the foster dog become lost, stolen, aggressive, seriously ill, or die. The foster dog will always wear a CCFR ID tag and a rabies tag (if provided). I understand that should I decide within the first 14 days of foster care or before the dog is posted for adoption to permanently adopt the CCFR foster dog in my care, I will pay CCFR the adoption fee and complete all appropriate paperwork.

    I certify that I am at least 21 years of age and that I will be solely responsible for the care and wellbeing of any dog that I foster care for CCFR. The above information is correct and does not contain false information. I agree to let CCFR check all references and understand that if my application does not meet the requirements of CCFR it may disqualify me from fostering with this organization. Any misrepresentation of the true facts in this foster home application will invalidate the foster home agreement and will give CCFR the right to immediately reclaim the foster dog.
