We're so glad your child is joining us for Chorister Camp this year! Complete one registration for each child in your family. Do you have a friend who would love Chorister Camp? Feel free to forward this form to anyone you think would be interested. We love visitors!
· Chorister Camp participants are rising 3rd- rising 12th grader.

· Chorister Camp is 9 am- 4 pm. If this schedule is a barrier for you, please contact us and we will make a plan for your family.

· Before-care is available starting at 7:30 a.m. After-care is offered 4-6pm. We ask for a donation of $40 per child for after-care. Scholarships are available- we want everyone to participate!

· Before-care is free to participating families.

· After-care costs $40 per day for the first child, $20 per day for each additional. –

· Donations for Chorister Camp can be made on this form or at the event. Payments for Before and After-care will be made at the event.

· You can choose which days you will attend Before and/or After-care and will be charged for only the days you attend.

· There is no flexibility about After-care pick up time. 6:00 is a strict deadline and a charge of $20 for every additional 5 minutes will be added except in cases of true emergency. - Our professional nursery staff are warm, welcoming, and highly skilled.

· Training is required for all volunteers; background checks are required if you are 17 or older.

· We eat lunch together from 12-12:30pm but a snack earlier in the morning will be provided.

· You will receive more details about how to participate in Chorister Camp as the event draws closer.

We will do everything we can to help your child engage meaningfully in Chorister Camp in ways that are safe and healthy for them, other campers, and our staff. If behavior problems arise that our staff cannot resolve, we will discuss them with you and create a plan together with you and your child. If your child cannot adhere to the plan and we have continuing problems, we reserve the right to exclude them from attendance for the remainder of the week. If your child is experiencing problems with the behavior of other campers or staff, we hope you will tell us immediately so we can address the situation and make sure everyone is having the best possible week at Chorister Camp.*

I understand and acknowledge the behavior management rules
I hereby indemnify and hold Saint Mark United Methodist Church from and against any liabilities that may be incurred during Chorister Camp (including Before and After-care if applicable). I also authorize, in the event that I cannot be personally contacted, Saint Mark united Methodist Church and/or its agent to authorize any medical treatment for the heath/welfare of my child in connection with any accident or similar emergency. I understand that I am responsible for any charges that may be incurred for such medical treatment. I also understand that my child’s failure to abide by the Chorister Camp safety rules and instructions may result in his or her being sent home. This release is valid until July 21st, 2025. We need your help to make Chorister Camp happen!

Ask any of our planners or volunteers- Chorister Camp is fun! Watching the kids grow in faith, build relationships, and have a blast throughout the week is an experience you'll never forget. If it seems intimidating, please don't worry! Our volunteer roles are designed so that when each person does their job, the burden doesn't fall heavily on anyone.

About Donations:

Donations per camper to VBS are much appreciated but ultimately optional.

Please give as you are able.

Before Care (starting at 8 am) is included as part of VBS, though we do pay our Before Care leaders.

After Care payments are not optional, though scholarships can be arranged on a case-by-case basis.

We want everyone to participate! That said, we rely on the generosity of those who are able to give to make this program possible. Please prayerfully consider your donation, and thank you in advance.
