Brightup After School Hub Registration
Welcome! Our Brightup After School Hub is designed for children aged 4 to 12, running Monday to Friday from 3:00 PM to 6:00 PM. Each day includes School Pick-up, Enrichment Activities (Art, Music, Math, Language Learning), Time for Snack, Homework, Reading, Board Games and Quiet Play.

Monthly Rates (HST and nut-free snacks Included):
(Example based on a month with 20 school days)

5 days/week: $495
4 days/week: $426
3 days/week: $348
Drop-in / Single-day option : $30/day (only if spots are available)

*Fees are collected monthly and calculated based on the actual number of school days in that month. All rates include HST.

Medical Consent & Release
I authorize the staff of ArtiSonata Studio to administer basic first aid and seek emergency medical treatment for my child if needed during camp hours. If I cannot be reached, I permit my child to be transported to the nearest medical facility and treated as necessary by qualified professionals. I understand that I am responsible for any medical expenses incurred and release ArtiSonata Studio, its staff, and affiliates from all liability related to medical emergencies or injuries during program participation.
Photo & Media Consent
By enrolling in our program, I give permission for photographs and/or videos of my child to be taken during camp activities for the purpose of documentation, promotional materials, and social media use by ArtiSonata Studio. These images may be used on the studio’s website, printed flyers, or social media platforms.No names or identifying information will be published without additional written consent.
Waiver and Release of Liability
While ArtiSonata Studio takes every reasonable precaution to ensure a safe and supportive learning environment, I understand that participation in lessons and activities may involve certain risks, including minor injuries, accidents, or allergic reactions. I acknowledge that my child’s involvement is voluntary and that I am responsible for notifying the studio of any relevant health concerns.
