Brightup After School Hub Registration
Welcome! Our Brightup After School Hub is designed for children aged 4 to 12, running Monday to Friday from 3:00 PM to 6:00 PM. Each day includes School Pick-up, Enrichment Activities (Art, Music, Math, Language Learning), Time for Snack, Homework, Reading, Board Games and Quiet Play.

Monthly Rates (HST Included):
(Example based on a month with 20 school days)

5 days/week: $495
4 days/week: $426
3 days/week: $348
Drop-In / Single-Day Option : $30/day (Only if spots available)

*The fee is collected monthly and is calculated based on the actual number of school days in that month. All rates include HST.

Medical Consent & Release

Waiver and Release of Liability
While ArtiSonata Studio takes every reasonable precaution to ensure a safe and supportive learning environment, I understand that participation in lessons and activities may involve certain risks, including minor injuries, accidents, or allergic reactions. I acknowledge that my child’s involvement is voluntary and that I am responsible for notifying the studio of any relevant health concerns.*

I agree to release ArtiSonata Studio, its staff, and instructors from any liability for injuries, losses, or damages that may occur during lessons or while on studio premises. I accept full responsibility for my child’s participation and understand that the studio is not liable for unforeseen incidents.
