This form should take you about 10 minutes to complete. ☕️ Kindly fill it out when you can provide all the details accurately. I am unable to book appointments if the information or photos are insufficient, as I need them to properly estimate the amount of time required for the appointment.
Before filling out this form, please make sure you have a fairly specific idea of what you are looking to get tattooed, and that you have researched my work and are familiar with the style I tattoo in. I will use the information submitted in this form to design your tattoo.
All requests will be considered but not necessarily chosen. Please know, this does not mean your idea is not good, but rather, I may not feel I am the best artist to execute the tattoo you are wanting. All requests WILL be replied to regardless. Booking information will be sent in the reply via text or email.
PLEASE NOTE:
Multiple Tattoo Requests: If you are wanting to submit more than one tattoo request, please fill out one form per tattoo.
Matching or Multiple People Requests: If this is a request for matching tattoos for multiple persons, please have one designated contact person fill out one form for the tattoo.
• My hourly rate is $180/hr + GST.
• Deposits range from $100 - $180 depending on the appointment and are non-refundable. For more details on this please visit www.tatteredlacetattooco.com. All deposits are processed through Square. I will NEVER request an e-transfer or credit card number in an email to pay a deposit.
Lastly, PLEASE be aware that the ONLY email you will be contacted by is 'bookings@tatteredlacetattooco.com'. Scammers may try to contact you using a similar email address (watch for spelling/contact).
