Waiver & Release of Liability

As the parent/legal guardian of the minor named above, I understand and acknowledge that participation in baseball tryouts involves inherent risks, including but not limited to physical injury, illness, or property damage. In consideration of my child’s participation in tryouts organized by Tennessee Tomahwaks, I agree to the following:

1. Assumption of Risk

I understand that baseball is a physically demanding sport and involves potential risks, including collisions, falls, and other unforeseen incidents. I voluntarily assume all such risks on behalf of my child.

2. Waiver and Release

I, for myself and on behalf of my child, heirs, personal representatives, and assigns, hereby release, discharge, and hold harmless Tennessee Tomahawks, its coaches, organizers, volunteers, facility owners, and affiliates from any and all liability, claims, demands, or causes of action related to personal injury, illness, or property damage arising from participation in the tryout.

I release Tennessee Tomahawks Baseball and any person(s) connected with Tennessee Tomahawks Baseball of any responsibility from accident or injury incurred as a result of his participation in any and all activities, including transportation to and from any Tennessee Tomahawks Baseball functions or activities.

3. Medical Authorization

In the event of an emergency, I authorize Tennessee Tomahawks staff or volunteers to secure necessary medical treatment for my child. I understand that I am financially responsible for any such treatment.

I certify that my child has been given a physical examination and is physically able to participate in all related activities. Please notify in writing any prescribed medications and / or physical conditions of which the Tennessee Tomahawks Baseball coaching staff should be aware of are listed below.

4. Behavior & Conduct

I understand that all players and spectators are expected to conduct themselves in a respectful and sportsmanlike manner. Any behavior deemed unsafe or disruptive may result in removal from the tryout without future eligibility.
