Creative Arts & Group Experiences Colorado LLC Liability Waiver

THIS IS A RELEASE OF LIABILITY, READ BEFORE SIGNING.

This agreement releases Creative Arts and Group Experiences Colorado LLC, from all liability relating to injuries that may occur on site and/or during one of the events. By signing this agreement, I agree to hold Creative Arts & Group Experiences Colorado LLC entirely free from any liability, including financial responsibility for injuries, regardless of whether injuries are cause by negligence.

I also acknowledge the risks involved in this activity. These include but are not limited to injury, psychological, emotional, spiritual, and physical distress, and/or death. I swear that I am participating voluntarily, and that all risks have been made clear to me. Additionally, I do not have any conditions that will increase my likelihood of experiencing injuries while engaging in this activity.

I also agree to reimburse Creative Arts & Group Experiences Colorado LLC for damages I inflict on the property or any material goods owned by Creative Arts & Group Experiences Colorado LLC, intentionally or un-intentionally during the course of the event, at all cost deemed appropriate by Creative Arts & Group Experiences Colorado LLC., owners or staff. I also acknowledge that Creative Arts & Group Experiences Colorado LLC, staff reserves the right to refuse anyone for any reason, and at any time during the event, without refunding any portion of the cost of the equipment and services provided.

By signing below, I forfeit all right to bring a suit against Creative Arts & Group Experiences Colorado LLC for any reason. I will also make every effort to obey safety precautions as listed in writing and as explained to me verbally. I will ask for clarification when needed.

For Parents/Guardians of a participant of minor age (under 18).

This is to certify that I, as guardian with legal responsibility for this participant, do consent and agree to his/her release as provided above of all the Releases, and, for myself, my heirs, assigns, and next of kin, I release and agree to indemnify and hold harmless Creative Arts & Group Experiences Colorado LLC and it’s owners, employees, representatives, and agents from any and all liability incidents to my minor child’s involvement or participation in the activity as provided above. EVEN IF ARISING FROM THE NEGLIGENCE OF Creative Arts & Group Experiences Colorado LLC to the fullest extent permitted by law.

The information collected will be used to help contact any individual that has participated in an activation provided by Creative Arts & Group Experiences Colorado LLC. All information is stored for a period of (30) days after submitting the laibility waiver to ensure that it is available in case of incident. This information is NOT sold or released to any individual or 3rd party after submission.
