    If you submit with a Quiz Score of 17 or less, you will not receive credit. You will need to review the PTA Basics PowerPoint and retake the PTA Basics Quiz until you receive a passing score of 18 or more. 

    You can save your current progress and return at a later time to complete the quiz but you will be prompted to create a (free) Jotform account once you click "Save & Return". When returning to complete the quiz, log in to your Jotform account and continue where you left off. 

    You DO NOT need to make a Jotform account if you complete and submit in one sitting. Otherwise, to save your work and revisit later, you will need to create a (free) Jotform account.
