  • I acknowledge that I have truthfully answered all the questions in this application. I hereby release, and agree to indemnify, defend and hold harmless Dulce Vida Animal Rescue, its directors, employees, and volunteers, and its and their heirs, successors, assigns, and personal representatives, from and against liability for any injuries, damages, liabilities, losses, judgments, cost or expenses whatsoever (the "Losses"), which I might suffer or sustain.

    and we/l assume such risks. Tetanus Information: I understand that because I may handle animals, it is important to discuss the tetanus vaccine with my physician. I release Dulce Vida Animal Rescue from all responsibility that may occur because of my not pursuing this matter further and I understand whatever decision I make is at my own risk. I have read, understand and agree to the above tetanus information. I am aware of the potential that certain zoonotic diseases can be passed along to humans (ex: scabies, ringworm, etc, and I release the Dulce Vida Animal Rescue from, and waive all claims and liability against Dulce Vida Animal Rescue for, any and all medical expenses related to or arising out of treatment for myself or any member of my household who may contract such diseases. On behalf of myself, my children, my heirs, assignees, guardians, personal and legal representatives and executors, I hereby release, discharge, indemnify and hold harmless Dulce Vida Animal Rescue, its officers, directors, and employees for any and all claims, demands, losses, costs, liabilities, damages, personal injuries and property damage, connected with my or my child's volunteer services to Dulce Vida Animal Rescue whether caused directly or indirectly by any negligence (active or passive) attributable to Dulce Vida Animal Rescue, its officers, directors or employees.
