If you have any other questions or concerns, please send me a message! NEW CLIENTS ONLY. IF YOU ARE AN EXISTING CLIENT PLEASE SEE OTHER FORM. !!! Please note !!! Payment is due the day of your appointment, a late payment will result in an extra $15 fee added. A cancellation fee is also in place, if the appointment is cancelled within the 12 hr window before your appointment you will be required to pay a $15 appointment fee. If it’s cancelled in less than 12 hr, the whole appointment will be required.
