This form is required to confirm your mele and finalize your entry for the 16th Annual George Naʻope Hula Festival. Judges rely on the information you provide to understand your interpretation and properly evaluate your performance.

Fact sheets are due by June 30, 2025

A separate fact sheet is required for each performance — this includes every solo and group entry in each division.

To ensure consistency, this is the only form that will be accepted for fact sheet submissions. Please do not submit forms from other competitions or personal formats, as they will not be accepted. All entries must be submitted using this official online form.

- Be sure all Hawaiian text includes proper diacritical marks.
