A Field level Hazard Assessment (FLHA) is performed before work starts at a site and at a site where conditions change or when non-routine work is added. Site-specific hazard assessments check for the introduction of any unexpected hazards, or hazards for which additional controls may be needed.
Any hazards identified during a site-specific hazard assessment must be addressed right away, before work begins at the location.
If a site-specific hazard assessment recognizes a hazard that was overlooked by the formal assessment, the formal assessment should be updated to include it.
Steps to fill a FLHA:
1. The supervisor or a designated worker will tour the site at the beginning of the
workday and identify hazards on site.
• Health hazard: A physical, chemical, biological or psychological hazard which
may cause acute or chronic health effects to exposed workers (ex. Noise, dust,
heat, ergonomics, etc.)
• Safety hazard: A substance, process, action or condition that may endanger the
immediate safety of the workers (ex. chemical burns, shear points, slip and
falls etc.)
2. The supervisor or the designated worker will then fill a FLHA and in a brief
meeting share its content with the crew before work starts.
3. All worker on site will then sign the FLHA.
4. If a new hazard is identified during the day, an update will be made to the current FLHA or a new one will be created.
5. If a new worker joins the crew during the workday, he will review the site FLHA and sign it
A FLHA:
• Is mandatory at least one per day and at least one per crew
• Is the responsibility of the supervisor
• Must be sign by every worker on site
• Must be kept on site and available to anyone while it is valid
• Must be review and approved by a manager
• Have the controls and decision on it respected and followed by everyone on site
