I understand that the group will travel by transportation provided by Temple or private automobile and will be accompanied by staff and Temple parents. Throughout the year, Temple Beth Or's members are photographed and video recorded as they participate in Temple activities. The photos and videos are used in Temple-related promotional and advertising materials disseminated to the public through newspapers, magazines, our website, social media, and other channels deemed appropriate and safe by Temple Beth Or. Unless otherwise indicated below, I/we give Temple Beth Or permission to use, edit, reproduce, and publish photographs, video, and audio of myself/us, both while the I/we belong/s to Temple Beth Or and thereafter in perpetuity. This authorization permits the use of photographs, video, and audio, in any medium or form of distribution, including, but not limited to, the Temple Beth Or website, newsletters, brochures, media outlets, promotional materials, or in any other manner as determined by the sole discretion of Temple Beth Or.

Payment
Temple Beth Or believes that Jewish education should be accessible to all. In an attempt to keep the tuition fees equitable for all, the Religious School offers a multi-tier fee structure. Members of Temple Beth Or have the option to choose which tuition rate works best for their family. Full Tuition (Actual Cost) reflects the operating cost to educate each child and provide quality, innovative programming. The subsidized rates (Standard Fee - 50% of cost, and Increased Fee - 75% of cost) represent the commitment of the congregation to our religious school with substantial underwriting from the Temple’s operating budget.

● Scholarship money is available for member families not able to pay our Standard Fee. No member family will ever be turned away for inability to pay. Please fill out the scholarship portion of this application to be considered.

● Tuition for nonmembers is at the Full Tuition (Actual Cost) rate. No scholarships are available for these students. This must be paid in full on August 1. This tuition is not tax deductible. For information on membership, contact the TBO office or VP of Membership at 425-259-7125 x1.

Note to Members: Standard Fee tuition is paid for services received, is not tax deductible. Any additional Religious School donations or additional monies paid towards Full Tuition or Increased Fee tuitions are tax deductible.

Please complete this form to the office before or by August 1. Students are not registered until these forms are properly completed and on file in the office. Fees must be paid as noted below, unless arrangements have been made with a member of the Finance Committee.

***EARLY BIRD REGISTRATION: ALL REGISTRATIONS RECEIVED BY JUNE 27 WILL RECEIVE A $50 EARLY BIRD DISCOUNT. THIS WILL BE ADDED TO YOUR STATEMENT.
