In consideration of being allowed to enter the play area and/or participate in any party and/or program at Mini Mocha Play Cafe LLC, the undersigned, on their own behalf and on behalf of the participant(s) identified below, acknowledges, appreciates, and agrees to the following:

1. Acknowledgment of Authority
I represent that I am the parent or legal guardian of the participant(s) named below, or that I have obtained permission from the parent/legal guardian to execute this agreement on their behalf.

2. Compliance with Rules
I agree that the participant(s) and I shall comply with all posted safety signs, rules, and verbal instructions as conditions for participation in any free play, party, and/or program at Mini Mocha Play Cafe LLC.

3. Waiver and Release of Liability
I understand that there are inherent risks of injury or illness, though minimal, in participating. I, for myself and for the children, and on behalf of our heirs, assigns, personal representatives, and next of kin, hereby waive, release, and discharge Mini Mocha Play Cafe LLC and its directors, officers, agents, employees, owners, and representatives from any and all losses, damages, injuries, expenses, and claims — including, but not limited to, those caused by negligence — for personal injury, death, or loss or damage to person or property.

I agree to indemnify and hold harmless Mini Mocha Play Cafe LLC from any liability, losses, claims, and damages resulting from the participation of the children in activities at the Cafe.

4. Assumption of Risk
I acknowledge and accept all risks — known and unknown — associated with participation in Mini Mocha Play Cafe programs, parties, and/or use of the play area, including risks arising from the negligence of other participants.

If I observe any hazard during our participation, I will immediately report it to the nearest Mini Mocha Play Cafe employee or official.

5. Health Acknowledgment
I certify that I and the child(ren) have no illness or injury that may affect the safe enjoyment of Mini Mocha Play Cafe LLC by us or other guests.

6. Personal Property
Mini Mocha Play Cafe LLC is not responsible for any lost or stolen items.

7. Media Release
I understand and agree that Mini Mocha Play Cafe LLC may take photographs, videos, or other recorded media (excluding private areas like restrooms) at any time.

I grant full rights to use my likeness and the likeness of the child(ren) in such media, in any publication (including web-based), without payment or other consideration. I waive any right to royalties or compensation and assign full copyright of the media to Mini Mocha Play Cafe LLC, its licensees, or assignees.

8. COVID-19 and Illness Acknowledgment
Mini Mocha Play Cafe LLC cannot prevent exposure to or the spread of COVID-19 or other illnesses. By entering the premises, I acknowledge and accept the risk of possible exposure, contraction, or transmission.

9. Choice of Law
I understand and agree that the laws of the State of Wisconsin shall govern this agreement.

I HAVE CAREFULLY READ AND FULLY UNDERSTAND ALL PROVISIONS OF THIS RELEASE AND FREELY AND KNOWINGLY ASSUME THE RISKS AND WAIVE MY RIGHTS AS DESCRIBED ABOVE.
