Medical Treatment and Liability Release

If needed, I give permission for my child to be evaluated, diagnosed, treated, &/or given medication in accordance with standard medical practice by licensed medical personnel. I understand that attempts to contact me, if necessary, will be made.

I give permission to Holy Spirit Parish, volunteers, & staff of all responsibility and consequences that may arise as a result of this treatment. I will not hold Holy Spirit Parish, all volunteers, chaperones & staff, liable in the event of injury or illness.

I agree to accept any & all financial responsibility as a result of scheduling medical treatment.

I understand that Holy Spirit Parish, volunteers, drivers, & staff will not be held liable for accidental harm, negligence, or if my youth fails to comply with regulations, & that any infraction of these rules may result in immediate dismissal from the event with transportation costs at your own expense.

The parents or guardians submitting this application have legal custody over the child.

For safety reasons and the good health of your child, attending Bible Camp is fully considered. However, in the event of an accident or sickness, the Parish Staff and volunteers at Holy Spirit Parish are hereby released from any liability for any harm arising to our child while attending Bible Camp events.

All children attending must have a valid health card provided by the Provincial Health Insurance or equivalent medical insurance.

In the event of a child's illness, parents/guardians will be notified, and a request will be made to pick up your child. To my knowledge, my son/daughter is in good health and is physically and emotionally capable of participating in these youth ministry events.

Good hygiene and respect for personal space will be encouraged. If your child is sick, has a fever for over 24 hours, please do not bring them and notify us.
