Assumption of Risk I, the undersigned, understand and agree that participation in any fitness activity involves inherent risks, including but not limited to musculoskeletal injuries, heart-related conditions, and accidents. I knowingly and voluntarily assume full responsibility for any and all risks, whether foreseen or unforeseen, associated with my participation in personal training and fitness activities provided by [Everyday Fitness12]. 2. Health Declaration I certify that I am physically able to participate in fitness activities and have either received clearance from a qualified medical professional or choose to participate at my own risk. 3. Waiver & Release of Liability In consideration of being allowed to participate in fitness training with [Everyday Fitness12], I hereby waive, release, and discharge [Irving Wright], [Everyday Fitness12], and any of its affiliates, employees, or contractors from any and all claims, demands, causes of action, or liability for injury, death, or damage arising from my participation, except in cases of gross negligence or willful misconduct. 4.
