  • I have received a copy of the GCISD Extra/Co-Curricular Activity Pledge and understand that I will be held accountable for my behavior and will be subject to the disciplinary consequences outlined in the Pledge.

  • I, and on behalf of my personal representatives, successors, and assigns, hereby agree to release, indemnify and hold harmless the District, its trustees, employees, and agents from all claims made by third parties against it or them that result from my child’s actions on the trip.

     

    As per Board Policy FMG(LOCAL), students who participate in school-sponsored trips shall be required to ride in transportation provided by the school to and from the event. Exceptions may be made if the student's parent/guardian presents a written request to the principal before the scheduled trip, requesting that the student be allowed to ride with the parent, with another person designated by the parent, or that the student be allowed to drive him or herself to the event.

     

    The District shall not be liable for any injuries that occur to students riding in vehicles that the District does not provide. I, and on behalf of my personal representatives, successors, and assigns, hereby agree to release, indemnify and hold harmless the District, its trustees, employees, and agents from all claims made against it or them that result from my request to use alternate transportation.

     

    I understand that the District, its trustees, employees, and agents are not waiving any sovereign or governmental immunity that it or they have under Texas law.

     

    I have read and understand this information and sign it voluntarily with full knowledge of its significance. I also understand that if I choose for my child not to participate in this field trip that an alternate activity will be provided for my child at the campus.

  • This Handbook is distributed to all band students and parents. The information is intended to make your Band experience as positive and fulfilling as possible. Both students AND parents are expected to read the Handbook.

  • This is to certify that I have received and read the Heritage Middle School Band Handbook, agree to all terms, and will observe all guidelines found therein and understand how it applies to myself/my student.*

    Student:

    I hereby agree to provide the very best care of the instrument while in my possession. I understand that I/my parents will assume the financial responsibility of repairing or replacing the instrument(s) should it become damaged, lost, or stolen while in my care. It is recommended that the borrowing student/parent photograph or otherwise document existing scratches, dents, or other markings on the instrument and case upon assignment. Directors review and record instrument condition annually prior to assignment for student use.

     

     

    Parent/Guardian:

    I acknowledge my responsibility in the care of the GCISD owned instrument(s) issued for my child’s use. I will assume the financial responsibility of repairing or replacing the instrument(s) by an GCISD authorized vendor should it become damaged, lost, or stolen while in my child’s care.

     

    School Owned Instrument Fees will be assessed and collected through Skyward.

  • Band Fees

  • Band fees are collected at the beginning of the school to avoid requesting payment for various items throughout the year, and because the material items covered are provided at the beginning of the year.  

     

    This fee covers:  initial band t-shirt, contest entry fees (All-District, All-Region, UIL, North Texas Festival of Distinction, adjudicator honorariums, binders, dividers, pencil pouches, 2 pencils & highligher, meals throughout the year, field trips, and more! 

     

    We will also be charging a $65 uniform fee for Advanced Band students (Honors, Symphonic, and Concert Bands) that need a formal concert uniforms. This will provide girls with a uniform black formal concert dress and boys with a uniform black concert shirt and pants, and is required for all advanced band students. Once purchased, this uniform belongs to the student. :)

            Join the Booster Club! Proceeds raised are used to support our students and improve band programs. We are proud of our many contributions which provide lesson scholarships, clinicians to improve performance levels, enrichment funds, college scholarships, and band director support for various band programs. These endeavors are made possible because of your generosity by becoming a member of the Booster Club and through concession sales at football games.
