Indemnity Form & Access:
1. The right of access to the Cardamom Kitchen (the “Business”) is reserved at all times.
2. All persons who enter the premises of the Business do so completely at their own risk.
3. All persons, parents, and parties to the agreement that enter the premises of the Business agree to observe and obey the rules of the Business and confirm that all rules are incorporated in this agreement.
4. No children will be accepted at the Business Kids Cooking Class before the Indemnity form is signed electronically or completed manually, signed, and made available to the Business.
I/we declare as follows:
- I/we am/are aware that the Business Cooking classes are an extramural activity that will require practical activities.
- I/we understand that although the Business representatives undertake to take all necessary steps to ensure your child's safety when in our care, it does not constitute a guarantee of the safety and welfare of the child at all times.
- I/we hereby grant permission for my/our child to participate in any official business activity at the named location.
- In case I/we wish to cancel this permission or in case of any specific instruction regarding participation I/we undertake to inform the Business Owner (Nisreen Arend) in writing.
I/we further grant permission that:
- In case of transport being needed in any case, like for example in case of emergency, I/we grant permission for my/our child to be transported by a mandated and specific person.
- In the case that medical treatment is needed during an emergency and the Business representatives have exhausted all reasonable methods of contact with me/us, I/we grant the Business Owner (Nisreen Arend) the right to authorize any necessary treatment.
- I/we declare that according to my/our knowledge my/our child is physically fit to participate in all activities offered by the centre and that he/she is in good health. I/we declare further that we have informed the centre of any physical disabilities that may affect my/our child’s daily interaction.
- I/we indemnify all members of the Business against claims or responsibility that may occur after the loss damage to property, physical injury, illness, or death that may affect me/us or my/our child that pertains to my/our child’s participation in the activities of the Business.
- I/we agree that I/we will be held liable for the payment of any expenses or medical costs in the case of injury to my/our child.
- I/we take note that in the case of my/our child’s participation in extramural activities, the Business cannot be held liable for my/our child’s health and safety.
- I/we will consider giving consent for my/our son's/daughter's pictures to be used on the Business website and their Social Media accounts ONLY.
