    Basic Registration Info

  • This Form is divided into 5 Sections. 

    Each Section Number 1-5 will appear in large bold
       at the top of the section in green lettering   

    Section 1: Basic Registration Info

    Section 3: Detailed info about meals and lodging for each person
    Section 4: Payment Info, Comments and Concerns
    Section 5: Submit and Registration is complete 

  • Register before May 23rd if attending in person. 

    After May 23rd,  you must contact the Registrar Eric Wolff.

  • Tips for filling out this registration form:

    • A little red star * to the right of a field description means that it is required. When you  try to move on to a new section using "Next", you will be stopped until you fill in all the required data fields. 

    • Hover over any input field and there may be additional helpful information about that field.

    • Clicking the keyboard End key will move you to the end of the section. 
    • Clicking the Home key will move you to the start of the section.
    • Use the   and    icons to jump back and forth between sections. These icons are found only at the end of each section.
  • IF everything goes well, when you are done here,
    you will receive a "Thank You" page in this form
    with a big green check mark,

    Family Info

     

  • If you are requesting a room, there are 2 beds in each room, 3 room options and a 4th option for no room.

    1. (Shared). For one person to share a room with another person to be chosen for you by the registrar.
    2. (Single). For the use of the entire room by one person
    3. (Family). For 2 beds for 2 of your family and floor space for 2 more.
    4. (Other). No room required.

     

  • There will be an option later in this form to identify children who will sleep on the floor.

    There will be an option later in this form to request a specific person be roomed with you if they attend and put in their own registration.

  • Family Contact Information

  • Family Contact identifies a person who will be attending, if a person won't be attending sessions such as a parent or a guardian, and there are children attending who require that an equivalent person be identified to the registrar and youth leaders. This person would be contacted in the case of an emergency or other problems related to the youth in this regisitration. This person would also share responsibility for paying the final bill. If there is a guardian attending, this person info can be left mostly blank. 
  • Family Contact identifies an off campus guardian of youth in this request.
    The email address is also where your successful registration verification will be sent, so it is required. 
  • If a child or youth is attending without their parent(s) or legal guardian also attending, we expect you to have a conversation prior to Sessions with Children's Program Clerk Hannah Branson or Youth Services Committee Clerk David Hyde.   This conversation will be initiated by your registration. They will contact you.

    • You may ask to be roommates with specific people and indicate this request in Section 3. There are no gender restrictions on shared rooms by choice.

     FYI  - Cash, Check and Zelle cost the least for OVYM.

    Square options info, such as Credit Card, will not show below unless you choose "Pay using Square".

    Zelle HowTo info, will not show below unless you choose "Pay using Zelle".

    Square now offers quite a few options for payment. (Credit card, ApplePay, GooglePay, CashApp, and ACH). I would recommend only using a payment option if you are familiar with it, have used it before on this computer you are using now, or you will be prompted to download the required apps like GooglePay, CashApp, and Plaid (used by ACH). ApplePay will only work on Apple computers configured for ApplePay. Using options new to you may get confusing.

    Credit Card. Please be careful to enter your credit card info exactly as on your card, otherwise you will get an error.

    when you come to Summer Sessions: 

    • Bringing a smartphone or laptop. There may be individuals who are doing a Zoom session on their own. 
    • Cables, power supply and other electronic stuff you might want.
    • Flashlight, chargers
    • Bring all the linens, toiletries and extras you will need for yourself and your family. These will not be provided this year.
