As a legal guardian of participant(s) listed below, I recognize that participating at Fortress of Fun will involve active play and may include bouncing, jumping, climbing and sliding. Catastrophic injury, paralysis, or even death can result from participation in any physical activity. I consequently waive and forever release Fortress of Fun, staff, owners, agents, facility and equipment owners as well as any other parties from the responsibility or liabilities for deductibles, medical expenses, and/or other damages incurred by my child/children, myself, or other family members while visiting Fortress of Fun or while attending a Fortress of Fun event. I authorize the staff of Fortress of Fun to summon medical attention in the event of injury if the parent or guardian cannot be reached. Having read, understanding, and agreeing to the above, I give my permission for my child/children to participate in the activities held at Fortress of Fun.
