WILD ONES PLAYROOM LLC

THIS IS A RELEASE OF LIABILITY, READ BEFORE SIGNING.

I represent that I am the parent or legal guardian of the participant(s) below OR I have obtained permiison from the parent or legal guardian of the participant(s) below to execute the agreement on their behalf.

This agreement releases WILD ONES PLAYROOM LLC, their affiliates, officers, members, agents, employees, and other participants from all liability relating to injuries that may occur on site and/or during one of the events, programs, activies, parties, or use of play area. By signing this agreement, I agree to hold WILD ONES PLAYROOM LLC, their affiliates, officers, members, agents, employees, and other participants entirely free from any liabilities, damages, any and all claims, including financial responsibility for injuries, regardless of whether injuries are cause by negligence.

I also acknowledge the inherent risks involved in playing at WILD ONES PLAYROOM LLC and knowingly and freely assume all such risk, both known and unknown, for both myself and the participant(s) named below.

I, for myself and the participant(s) named below, will read and abide by ALL posted rules including, but not limited to: no running, no pushing, no rough play, no throwing any items, no climbing up the slide(s), no shoes allowed in the play area, wearing socks inside of the play area, minors being supervised by and parent/guardian at all times, sliding feet first down the slide(s), one child at a time going down the slide(s), no jumping off of anything. I understand that there is no food, drinks, or gum allowed inside of the play area at any time.

I understand that I am responsible for monitoring the participant(s) named below during the entirety of our visit to WILD ONES PLAYROOM LLC.

I acknowledge that WILD ONES PLAYROOM LLC staff reserves the right to refuse service to anyone for any reason. 

I acknowledge that this waiver, it's contents, and my signature is valid for one (1) year from the date I submitted on the waiver.

By signing below, I forfeit all right to bring a suit against WILD ONES PLAYROOM LLC,  for any reason. I will also make every effort to obey safety precautions as listed in writing and as explained to me verbally. I will ask for clarification when needed.

For Parents/Guardians of a participant of minor age (under 18) this is to certify that I, as guardian with legal responsibility for this participant, do consent and agree to his/her release as provided above of all the Releases, and, for myself, my heirs, assigns, and next of kin, I release and agree to indemnify and hold harmless WILD ONES PLAYROOM LLC and it’s owners, employees, representatives, and agents from any and all liability incidents to my minor child’s involvement or participation in any and all activies, events, parties as provided above. EVEN IF ARISING FROM THE NEGLIGENCE OF WILD ONES PLAYROOM LLC to the fullest extent permitted by law.
