SUMMER HIFZ 2025 PARTICIPANT WAIVER AND RELEASE OF LIABILITY

I, the undersigned, as the parent/legal guardian of the participant named above, acknowledge and understand that participation in the Summer Hifz 2025 ("the hifz program") hosted by the Islamic Centre of Markham ("the Centre") involves certain risks, which may include, but are not limited to, physical injury, illness, or accident.

In consideration of the opportunity afforded to my child to participate in the hifz program, I, on behalf of myself and my child, hereby release, discharge, and agree to hold harmless the Centre, its employees, volunteers, representatives, and agents from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, injury, or death, that may be sustained by my child while participating in the hifz program or while in, on, or upon the premises where the hifz program is being conducted.

I further agree to indemnify and hold harmless the Centre, its employees, volunteers, representatives, and agents from any loss, liability, damage, or costs, including court costs and attorney's fees, that they may incur due to my child's participation in the hifz program, whether caused by the negligence of the Centre or otherwise.
