In consideration of being allowed to participate in the Wrestling Seminar hosted at Eleven Elevation on June 13–14, 2025, the undersigned acknowledges and agrees to the following:
1. Assumption of Risk
I understand that participation in wrestling involves physical contact, intense exertion, and inherent risks of injury, including but not limited to: sprains, strains, fractures, concussions, dislocations, lacerations, and potentially serious bodily harm. I voluntarily accept and assume all risks associated with participation, both known and unknown, even if arising from the negligence of others.

2. Health & Physical Readiness
I affirm that I am in good health and physically able to participate in this wrestling seminar. I am solely responsible for monitoring my condition and will remove myself from participation if I experience any pain, dizziness, or other discomfort. I understand it is my responsibility to consult a physician prior to participating if I have any pre-existing medical conditions.

3. Release and Waiver of Liability
I hereby release, waive, and discharge Eleven Elevation, its owners, instructors, guest clinicians (including Arsen Aleksanyan and Valentin Kalika), volunteers, agents, and affiliated personnel from any and all liability, claims, or causes of action arising out of or related to my participation in this seminar. This includes, but is not limited to, personal injury, illness, property damage, or death, regardless of cause, including negligence.

4. Facility Conduct and Expectations
I agree to follow all rules and safety instructions given by the instructors and staff. I understand that unsportsmanlike or dangerous behavior may result in immediate dismissal from the seminar without refund.

5. Photo/Video Release
I give permission for Eleven Elevation to capture photos or video of the event and use my image in promotional materials, publications, and social media, unless I opt out in writing prior to the event.
