Liability Waiver and Release of Claims

New Beginning Worship Center – Summer Camp Program

By signing below, I, the undersigned parent or legal guardian of the child(ren) listed below, hereby acknowledge and agree to the following:

Voluntary Participation:
I understand that my child’s participation in the New Beginning Worship Center Summer Camp is voluntary. I acknowledge that participation in physical and recreational activities may involve certain risks, including but not limited to minor injuries, sprains, falls, or other unforeseeable incidents.

Assumption of Risk:
I willingly assume all risks associated with my child’s participation in the camp, whether known or unknown, including risks that may arise from the actions or negligence of staff, volunteers, other participants, or conditions on the premises.

Release of Liability:
I hereby release and hold harmless New Beginning Worship Center, its pastors, staff, employees, volunteers, representatives, and affiliates from any and all liability, claims, demands, losses, or damages arising out of or related to any injury, illness, or accident that may occur while my child is participating in the Summer Camp program, except in cases of gross negligence or intentional misconduct.

Medical Treatment Authorization:
In the event of an emergency, I authorize New Beginning Worship Center staff or volunteers to seek appropriate medical attention for my child. I understand that every reasonable effort will be made to contact me before such care is administered.

Commitment to Safety:
I acknowledge that New Beginning Worship Center has taken every reasonable measure to ensure the safety and well-being of all participants, including maintaining trained staff, first-aid supplies, structured activities, and emergency protocols.

Photo/Video Release (optional):
I grant permission for my child to be photographed or recorded during camp activities, and for these images to be used in church publications or social media unless I submit a written opt-out request.

 

Medical Authorization & Emergency Treatment Consent

New Beginning Worship Center – Summer Camp

As the Parent and/or Legal Guardian of the child named below, I hereby authorize the diagnosis and treatment by a qualified and licensed medical professional of said minor child in the event of a medical emergency which, in the opinion of the attending medical professional, requires immediate attention to prevent further endangerment of the child’s life, physical disfigurement, impairment, or other undue pain, suffering, or discomfort, if treatment is delayed.

Permission is hereby granted to the attending physician to proceed with any necessary medical care, including but not limited to minor surgical treatment, X-ray examinations, and immunizations, for the named child. In the event of a serious illness, major surgery, or significant injury, I understand that every reasonable attempt will be made to contact me as quickly as possible. This authorization is granted only after reasonable effort has been made to reach me.

Permission is also granted to New Beginning Worship Center, including its pastors, directors, camp staff, volunteers, and team leaders, to provide emergency first aid or necessary immediate care prior to the child’s admission to a medical facility.

This authorization shall remain in effect for the duration of the registered summer camp session unless revoked in writing by the parent or guardian.

I acknowledge that this release is executed freely and voluntarily, for the purpose of authorizing medical treatment under emergency circumstances to protect the health and safety of my child while participating in activities at New Beginning Worship Center Summer Camp.

Refund Policy - New Beginning Worship Center Summer Camp

We understand that plans may change. In order to best serve all families and manage staffing, supplies, and scheduling for our campers, we have established the following refund policy:

Cancellations & Refunds
Cancellations must be made at least 3 days prior to the start of your child’s registered session in order to receive a full refund.
If a cancellation is made the week of the session start (for example, on Monday), only 50% of the session fee will be refunded.

 

Cancellation Deadline:

1st Session: June 25, 2025

2nd Session: July 16, 2025

For example: If you cancel on the first day of Session One ($300), you will receive a $150 refund.

If the 2nd session was also paid for in advance, $150 will be refunded from the 1st session, along with $250 ($50 discount) from the second session, totaling a $400 reimbursed.

Why We Retain 50% for Week-Of Cancellations:
By reserving a spot, resources such as staffing, materials, meals, t-shirts, and space are allocated specifically for your child. When a cancellation occurs at the last minute, it is difficult to reassign that space to another camper, and the camp has already incurred costs. Therefore, 50% of the first week’s session fee is retained to cover those committed expenses.

BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
