Waiver & Release of Liability In consideration of being allowed to participate in fitness training with [Everyday Fitness12], I hereby waive, release, and discharge [Irving Wright], [Everyday Fitness12], and any of its affiliates, employees, or contractors from any and all claims, demands, causes of action, or liability for injury, death, or damage arising from my participation, except in cases of gross negligence or willful misconduct. 4. Indemnification I agree to indemnify and hold harmless [Business Name] and its agents against any claims or lawsuits arising out of my actions or failure to follow instructions during training sessions. 5. Emergency Medical Treatment In the event of a medical emergency, I authorize [Everyday Fitness12] to seek emergency medical treatment on my behalf. ⸻ Social Media & Website Consent. I grant [Everyday Fitness12] permission to take photographs, video, or audio recordings of me during training sessions for use in marketing, promotional materials, website content, or social media platforms. I consent to the use of my image, name, and likeness. I understand that no compensation will be provided for the use of these materials and that I may revoke consent at any time by submitting a written request.
