Shale Hollow is our primary location and will be prioritized, however we are open to using Blendon Woods as a satellite location.

Group OT (3-5 children with 1 OT and a volunteer student). Groups will be formed based on child ages/stages and family goals to provide a therapeutic experience for all children.
Please check ALL times that your child is available for group services at Shale Hollow Park.

Please provide any comments, concerns, or questions about scheduling. If you are requesting an alternate location for services (such as Blendon Woods/East side), please indicate that here. Priority scheduling will be give to clients who are able to attend sessions at Shale Hollow.
