Liability Waiver
ASSUMPTION OF RISK & RELEASE OF LIABILITY
I acknowledge that participation in Peak Elite Performance training programs, camps, and classes involves
physical activity and may carry some inherent risks, including but not limited to minor injuries (such as
sprains and strains) or, in rare cases, more serious injuries. I understand that Peak Elite Performance takes
reasonable precautions to ensure a safe training environment, but I accept full responsibility for my
participation (or my child’s participation) in these activities.
By signing below, I fully release and discharge Peak Elite Performance, its owners, staff, coaches, affiliates,
and facility partners from any and all claims, liabilities, or damages arising out of participation in these
activities, including those resulting from ordinary negligence. I understand that this waiver applies to all
present and future participation and that I am signing voluntarily with full awareness of its legal significance.
MEDICAL CONSENT & EMERGENCY TREATMENT
I confirm that I (or my child) am in good physical condition and able to participate in Peak Elite Performance
programs. I understand that in the event of an injury or medical emergency, Peak Elite Performance staff may
provide basic first aid and, if necessary, seek professional medical assistance.
I acknowledge that any medical expenses incurred are my responsibility, and I agree not to hold Peak Elite
Performance, its staff, or affiliates liable for any medical treatment or related costs. I understand that it is my
responsibility to inform Peak Elite Performance of any medical conditions, allergies, or restrictions prior to
participation.
PHOTO & VIDEO RELEASE
I give Peak Elite Performance permission to capture and use photos or videos of me (or my child) during
training sessions for promotional, educational, and marketing purposes, including on social media, websites,
and other materials. I understand that these images will be used professionally and without compensation.
If I prefer not to have photos or videos taken, I will notify Peak Elite Performance in writing before
participating.
ACKNOWLEDGMENT OF POLICIES
I agree to follow all rules, guidelines, and safety instructions set by Peak Elite Performance. I understand that
these policies are in place to ensure a safe and positive training environment for all participants. If I fail to
follow these policies, I understand that it may result in my (or my child’s) removal from the program without
a refund.
LEGAL AGREEMENT
I acknowledge that I have read, understood, and voluntarily signed this Liability Waiver & Release Form. I
understand that by signing, I am waiving certain legal rights and agreeing to participate (or allow my child to
participate) at my own risk.
