Informed Consent and Acknowledgment
I understand that participation in physical fitness activities—including but not limited to mobility training, strength training, and yoga—carries inherent risks of injury. These risks may include, but are not limited to, muscle strains, joint injuries, cardiovascular incidents, falls, or other health complications. You (each participant, member, guest and all participating family members) agree that if you engage in any NEVE activity related to “training” for any of the following activities such as strength training, general fitness and wellness, nutrition consultation, running, cycling, hiking, boating, skiing, skinning, indoor bouldering and climbing coaching, adventure racing (but not excluding those not specifically mentioned) you do so entirely at your risk. You agree that you are voluntarily participating in these activities and assume all risks of injury, illness, or death.
I acknowledge the following:
● I have been advised to consult with my physician before beginning any program with NEVE Ltd.
● I am voluntarily participating in these activities with full knowledge of the potential risks.
● I am physically and mentally capable of engaging in mobility, strength, and yoga training sessions.
● I agree to communicate any discomfort, limitations, or health changes to my instructor or trainer immediately.
● I agree to follow all instructions to reduce risk of injury.
Waiver and Release of Liability
In consideration of being allowed to participate in training sessions and activities with NEVE Ltd, I hereby waive, release, and discharge NEVE Ltd, its directors, officers, employees, contractors, agents, and representatives from any and all claims or causes of action, known or unknown, arising out of or related to my participation, including but not limited to those resulting from negligence.
This waiver covers:
● Any personal injury, illness, or death
● Property damage or loss
● Any adverse effects related to exercise or the use of equipment
I understand that NEVE Ltd does not provide medical or accident insurance and that I am responsible for my own medical coverage.
Medical Authorization
In the event of an injury or medical emergency, I authorize NEVE Ltd to obtain medical assistance deemed necessary and understand I will be responsible for any associated costs.
Governing Law
This waiver shall be governed by and construed in accordance with the laws of the state/province of Colorado.
