Dodgeball Tournament Registration Form
6/28 - Sports Forum, Wichita, KS, 11AM-3PM. Guaranteed Ticket to Baseball Game for all contestants & Giveaway Jersey for championship teams. $500 Cash Prize. Final Two Teams will play in centerfield at 5:30pm at Equity Bank Park. All participants must sign participation waiver at Wichita Sports Forum prior to game play.
