PLEASE READ CAREFULLY. THIS IS A LEGAL DOCUMENT. BY SIGNING, YOU MAY BE GIVING UP CERTAIN LEGAL RIGHTS.
I, the undersigned volunteer, desire to participate in activities organized by Sunflower Seeds Ukraine (herinafter known as “the Organization”), including but not limited to packing and organizing pallets of humanitarian aid at a warehouse facility. I understand that participation is voluntary and involves physical activity in a working warehouse environment.
1. Assumption of Risk
I understand and acknowledge that volunteering in a warehouse may involve risks of injury, including but not limited to lifting injuries, trips and falls, use of pallets and warehouse equipment, and other hazards. I voluntarily assume all risks associated with my participation, whether known or unknown.
2. Release and Waiver
I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby release, discharge, and hold harmless the Organization, its directors, officers, employees, agents, volunteers, affiliates, sponsors, and partners from any and all liability, claims, demands, or causes of action for any injury, loss, or damage to person or property, including death, which may result from or arise out of my participation in this activity.
3. Medical Treatment
I authorize the Organization to provide or seek emergency medical treatment if I am injured or become ill while participating. I understand I am solely responsible for all costs related to such treatment.
4. Insurance
I understand that Organization does not provide health or accident insurance for volunteers. I am responsible for any medical or other costs incurred as a result of my participation.
5. Conduct and Safety
I agree to follow all safety instructions, wear appropriate clothing and footwear, and behave in a responsible and respectful manner at all times. I understand that failure to do so may result in my removal from the activity.
6. Media Release
I grant permission to the Organization to use photographs, video, or audio recordings taken during my participation for promotional, educational, or fundraising purposes.
7. Minors
(If under 18 years old) I certify that I am the parent or legal guardian of the minor named below and that I have the legal authority to sign this waiver on their behalf.
