Honesty & Truthfulness Certification Statement

As part of your application for employment with Skilled Professionals, and in accordance with internal policy, state law, and federal regulations, you are required to read, acknowledge, and agree to the following statement regarding honesty, truthfulness, and the sensitive nature of your submitted information.

1. Certification of Truthful and Accurate Information
By signing below, I certify that all information provided on my application, resume, submitted documents, background questionnaire, and in all phases of the recruitment and selection process is complete, accurate, and truthful to the best of my knowledge.

I understand that any misrepresentation, falsification, or material omission of fact may be grounds for:
- Disqualification from employment consideration,

2. Consent to Verification
I understand that the information I provide will be used to verify:

- Professional licenses and certifications,
- Driving records and other relevant credentials.

I understand that Skilled Professionals or its contracted third-party screening agency may contact any listed employers, references, government agencies, or law enforcement agencies as part of this verification process.

3. Immigration and Work Eligibility
I understand that all information provided will be used in compliance with the Immigration Reform and Control Act (IRCA) and may be verified through E-Verify or similar legal tools to confirm my lawful eligibility to work in the United States.

4. Confidentiality of Submitted Information
All information submitted as part of this process will be kept confidential and handled in accordance with applicable federal and state laws. Only authorized personnel involved in the hiring process will have access to sensitive data, and data will not be shared externally without appropriate legal authority or consent.

5. Acknowledgment and Agreement

Non-Disclosure Agreement (NDA)
Confidentiality & Non-Disclosure Agreement

This Agreement is entered into between the undersigned applicant ("Recipient") and Skilled Professionals LLC ("Company").

1. Confidential Information
The recipient agrees to keep confidential all proprietary, sensitive, and privileged information disclosed during the application process and employment. This includes but is not limited to: operational procedures, client data, security protocols, equipment specifications, and internal communications.

2. Restrictions
Recipient shall not disclose, copy, or disseminate Confidential Information to any third party without prior written consent from the Company.

3. Term
This Agreement remains in effect during and after employment, and indefinitely in cases of sensitive security information.

4. Enforcement
Violations of this agreement may result in disciplinary action, legal claims, or termination of employment.

Consent for Background Investigation

I, the undersigned, authorize Skilled Professionals LLC and its agents to conduct a thorough background investigation, including but not limited to:
- Criminal history checks (local, state, federal)
- Employment history verification
- Education credential verification
- Motor vehicle records
- Professional license verification
- Social media/public record scans (where allowed by law)

I understand this information is used solely to determine suitability for employment and that Skilled Professionals LLC complies with all applicable federal and state laws.

I release all persons and entities from any liability in connection with furnishing such information. I understand I have rights under the Fair Credit Reporting Act (FCRA) and may request additional details if a consumer report is used.

Employment and Records Release Authorization

I authorize any current or former employer, educational institution, law enforcement agency, or other relevant source to release any information about my qualifications, performance, disciplinary record, or employment history to Skilled Professionals LLC.

I further authorize Skilled Professionals LLC to verify the truthfulness and accuracy of the information I have provided on my application, résumé, or in interviews. This authorization shall remain valid throughout the hiring process and, if hired, during my employment.

A photocopy or digital copy of this authorization shall be considered as valid as the original.
