Acknowledgment of Risk Form 

I, the undersigned, recognize the element of risk in any adventure, sport, or activity associated with the outdoors. The  undersigned is fully aware of the risks inherent in hiking including  but not limited to falls, cuts, scrapes, shock, sunburn, dehydration, hypothermia, lightning strikes, animal encounters and other mild or serious conditions. I understand that the possible consequences of participating in this activity  include bodily injury, death, or damage or loss of personal property.  

I have the necessary skills and ability to participate in this activity conducted by Rooted & Wild Adventure Co. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releasees or others, and assume full responsibility for my participation.

I agree to abide by the rules or instructions given to the undersigned either verbally or in writing by Rooted & Wild Adventure Co. I further understand that Rooted & Wild Adventure Co. reserves the right to refuse to allow any person to  participate in this activity who is judged to be incapable of meeting the rigors and requirements of participation. If I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately. 

I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, hereby release, indemnify, and hold harmless Rooted & Wild Adventure Co., its officers, officials, agents or employees, other participants, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (releasees), from any and all claims, demands, losses, and liability arising out of or related to any injury, disability or death I may suffer, or loss or damage to person or property, whether arising from the negligence of the releasees or otherwise, to the fullest extent permitted by law.

I have read, understood, and accepted the terms and conditions in this agreement. My participation in this activity conducted by Rooted & Wild Adventure Co. is purely voluntary, and the undersigned elects to  participate in spite of the risks.
