93 Supreme Training - Liability Waiver & Release Form
Acknowledgment of Risk

I understand that participation in football training activities offered by 93 Supreme Training involves inherent risks, including but not limited to:

Physical exertion

Contact with other participants

Use of training equipment

Weather conditions

Potential for injury, including serious injury or death

I affirm that I and/or my child am/are in good physical condition and am/are voluntarily participating in these activities with full knowledge of the risks involved.

Medical Clearance & Responsibility

I certify that I and/or my child have been cleared by a physician for physical activity and have disclosed all medical conditions, injuries, allergies, or limitations. I agree to inform the coach of any changes to this information prior to each session.

Waiver & Release of Liability

In consideration for being allowed to participate in training sessions provided by 93 Supreme Training, I hereby:

Release, waive, and discharge 93 Supreme Training, its staff, coaches, and affiliates from any and all liability, claims, demands, or causes of action arising out of participation, including injury, illness, or property damage.

Agree to indemnify and hold harmless 93 Supreme Training against any claims, costs, or expenses resulting from my participation or the participation of my minor child.

Consent to Participate

By signing below, I confirm that I have read and understood this waiver. I acknowledge the risks involved and agree to the terms outlined above.
