Out of courtesy for the professional working with you and others scheduled before and after you, please arrive on time. If running late, just give us a call or text! We will still be able to accommodate your class/event, but please note that your full experience may be cut short if more than 10 minutes late.

Carisma Dance closely follows and enforces the Covid-19 precautions and guidelines as stated by the CDC and local authorities. We kindly ask that you stay home if feeling unwell and use hand sanitizer upon arriving and throughout your time together. Masks are totally ok any time.

All cancellations must be done by phone or e-mail no less than 48 hours before the time of your class (24 hours for online). No refunds will be issued if cancelled less that 48 hours (24 hours for online) as your image professional/and dance instructor has carefully reserved and prepared that time just for you.

In the event that your instructor needs to cancel or reschedule your consultation or dance class, you will be immediately contacted by phone and/or e-mail so please make sure that the information you provided is accurate.

IN-STUDIO DANCE CLASS RELEASE OF LIABILITY 2024.

For and in consideration of Isabel De Montiel ("Host") permitting the participant to enroll and participate in the bellow chosen dance classes. Participant (which includes, but is not limited to, you and every person, adult or child, in the location), by clicking "I ACCEPT" below, hereby voluntarily indemnifies, releases from liability, and holds harmless Host, any and all additional presenters (collectively "Presenters"), and the Class Provider for any accident, injury, illness, death, loss, damage to person or property, or other consequences suffered by Participant or any other person arising or resulting directly or indirectly from Participant's participation in the Live Stream or any in-person dance class. In the event that Participant is injured, Participant agrees to assume any financial obligation, either through Participant's personal health insurance, or through some other means, for any medical costs which Participant incurs. Host, Presenters, and Class Provider assumes no responsibility for any medical expenses, injury, or damage suffered by Participant in connection with the use of any facilities or services in connection with any Dance Classes taught.

PARTICIPANT SHOULD CONSULT THEIR PHYSICAN OR OTHER HEALTH CARE PROFESSIONAL BEFORE JOINING ANY DANCE CLASS. THIS IS PARTICULARLY TRUE IF PARTICIPANT HAS A HISTORY OF HIGH BLOOD PRESSURE OR HEART DISEASE, OR IF PARTICIPANT HAS EVER EXPERIENCED CHEST PAIN WHEN EXERCISING OR HAS EXPERIENCED CHEST PAIN IN THE PAST MONTH WHEN NOT ENGAGED IN PHYSICAL ACTIVITY, SMOKES, HAS HIGH CHOLESTEROL, IS OBESE, OR HAS A BONE OR JOINT PROBLEM THAT COULD BE MADE WORSE BY ACTIVITIES IN CLASS  IF THEIR PHYSICIAN OR HEALTH CARE PROVIDER ADVISES AGAINST IT.

IF PARTICIPANT EXPERIENCES FAINTNESS, DIZZINESS, PAIN OR SHORTNESS OF BREATH AT ANYTIME WHILE PARTICIPATING IN THE CLASS, PARTICIPANT SHOULD STOP IMMEDIATELY.

PARTICIPANT MUST BE 18 YEARS OF AGE OR OLDER TO ACCEPT AND PARTICIPATE IN ANY DANCE CLASS. PARTICIPANTS UNDER THE AGE OF 18 MUST HAVE THE CONSENT OF PARENT OR GUARDIAN.

I understand that I and all participating in the classes must have this release form signed before attending. I give permission to Image Concepts by Isabel De Montiel and Carisma Dance Academy to use pictures and or video of me, my likeness and/or my child for advertising purposes in print, promotional videos and on the website.

ALL CLASSES REQUIRE A MINIMUM OF 4 STUDENTS.
