    Registration Form - Return of the Circus 2025

  • Dear Parent / Guardian,

    We are very excited that we are running the Return of the Circus Holiday Bible Club, and we are so pleased that you would like your child or children to come.  It is open to all children aged 5 to 11.  It will cost £5 per child to attend.  “Brookdale Holiday Bible Club" is run by volunteers (all of whom have been Disclosure and Barring Service checked) at Brookdale Church, Ilfracombe from 9:30am to 12:30pm on Wednesday 22nd August to Friday 22nd August 2025. 

    There is also an all-age bar-be-que with face painting and other fun on Friday 22nd August at 12:30pm to finish everything off, which parents, grandparents etc. are all welcome to come to.

    Please complete this form. We cannot accept responsibility for children without this form being completed. If they come to Holiday Bible Club without a form having been completed they may be turned away and will remain the sole responsibility of you. The form is required so the leaders have the information they may need in the event of an emergency. 

    Please can we ask you to ensure that your child gets to club safely, and then gets home safely after club. The leaders cannot be held responsible for the safety of any young people outside club times.

  • Your privacy is important to us at Brookdale Church. Under data protection legislation, we need to let you know how we care for your information.
    We are collecting this information to enable the church to run the Holiday Club safely and ensure we can contact you in case of an emergency. Data Protection legislation allows us to process this information as we regard it as being in the church’s legitimate interest. If you are unable to supply the information requested then we will be unable to accept your child at our Holiday Club.


    The information you supply will be held in paper form, securely locked on the premises as well as online. Only the relevant leaders will have access to this information. We will not share it with third parties unless the law requires us to. You have the right to request access to your personal data that we hold.

    The forms will be destroyed once Holiday Club has finished with the following exceptions:

    1) If you have ticked the box asking us to keep you informed about future activities we think your child might be interested in attending. If this is the case we will retain your contact details for the sole purpose of notifying you of such events.

    2) The consent section for photographs and video will be retained for as long as we keep the images/video.

  • I give my consent as parent / legal guardian to the leaders of Brookdale Church Holiday Bible Club for my child named on this form to attend and participate in the normal activities of Brookdale Church Holiday Bible Club. In addition I give the leaders permission to act on my behalf, using the information provided on this form, should the need arise.

    If for any reason my child needs to leave club early I will provide written consent for this.
