    In order for the Board of Directors to determine how to proceed with this complaint, please answer all of the following questions. Your answers will be held in the strictest confidence.
  • If your complaint involves a breach of a by-law, it will be presented at the next scheduled Board meeting. You will receive a response within 10 days following that meeting.
    If your complaint concerns a policy violation (not a by-law), it will be handled by the office. Staff will escalate the matter to the Board of Directors only if necessary.

I consent to this collection and any subsequent use of this information by the Board of Directors, office staff, managment, lawyers or its agents. I also understand that this information may be disclosed to office staff, management, and laywers of the co-operative, the Board of Directors, and if an eviction decision based wholly or partly on this complaint form, to the Landlord and Tenant Board (if the eviction decision is appealed) and to a court.*
