I, as the parent/legal guardian/person responsible give my permission for the participant to participate in all of the normal activities (academic, tutoring, vocational, field trips, workshops, etc.) of the MSK EDGE Summer Camp. By signing this permission release form, you acknowledge and agree to the terms and conditions of participation in the MSK Community Center Summer Camp.

MSK will not sell or use the photographs or videos in a manner that would exploit or cause malicious representation toward participant or participant's families and associates.
