Cancellation Acknowledgment and Agreement By submitting this cancellation request, I confirm that I have read, understood, and agree to the Terms and Conditions of Bella Ballerina, including all tuition, cancellation, and payment responsibilities as outlined in my enrollment agreement.I understand and agree that:This cancellation is not finalized until the final 4-week billing cycle payment has been processed in full.My dancer may continue attending classes during this final billing period.Missed classes, personal dissatisfaction, a change in teacher, or my child’s disinterest in dance do not constitute grounds for immediate cancellation or refund.Any recital or event payments previously made may be applied toward outstanding tuition balances if applicable.I am responsible for resolving any billing questions directly with the studio and will not initiate a chargeback with my financial institution for any validly processed charges.If I am enrolled in the Dance-All-Year program, I am responsible for an additional $300 early termination fee.If I am enrolled in the Performance Team (BBPT) program, I am responsible for an additional $400 early termination fee.These fees are due per the terms of the mutually agreed-upon contract and are required to finalize early withdrawal from either program.I further understand that failure to fulfill these and any remaining financial obligations may result legal action and my account being turned over to a collections agency.
