Please read before starting your application for any programming you require a scholarship for.

Making Our Programs Accessible for Every Family

Thank you for your interest in one of the programs offered by Bernadette’s House, including the Blossom Mentoring Program, Brains & Beauty Summer Program, and other youth development initiatives.

We’re committed to making our programs accessible to all families and are proud to offer full and partial scholarships based on financial need and program eligibility.

Scholarships typically cover the cost of program participation fees but do not cover registration fees, field trips, transportation, or food, unless stated otherwise.

📋 Before You Begin
This application takes approximately 10–15 minutes to complete.
Please gather all required documents before starting the form.
Only one application per family is needed — include all participating children’s names and ages.

📤 Documents Required
To qualify for a scholarship, you must upload proof of one of the following:

If enrolled in a public assistance program, upload your current benefit letter for one of the following:

• SNAP
• TANF
• FARMS (Free and Reduced Meals)
• Medicaid

If claiming low-income status, upload one of the following:

• The last two pay stubs for each income-earning adult in the household
• Your 2024 IRS Tax Transcript if self-employed
• Proof of unemployment benefits
• If you are unemployed and not receiving benefits, you must upload documentation for one of the programs listed above.

👭 Sibling Discount
If registering more than one child, your family will automatically be considered for a 50% scholarship for all additional participants. Please note this on your form.

💌 Important Notes
Scholarship applications are reviewed on a rolling basis and are awarded based on available funding and need.
If your application is approved, you will receive a coupon code via email to use on the official program registration form.
Please do not complete registration until you receive your scholarship code.

Please read the following carefully. By signing below, you confirm your understanding and agreement with each statement:

1. I certify that the information provided in this application is true and accurate to the best of my knowledge. I understand that providing false or incomplete information may result in disqualification from scholarship consideration.

2. I understand that submitting this application does not guarantee a scholarship.
Scholarships are limited and awarded based on need, eligibility, available funds, and documentation provided.

3. I understand that I am required to upload supporting documentation.
This includes income verification or proof of enrollment in qualifying public assistance programs (SNAP, TANF, FARMS, Medicaid). Failure to submit required documentation will result in an incomplete application.

4. I consent to Bernadette’s House staff reviewing and verifying the information provided for scholarship determination. I understand that my information will be kept confidential and only used for program eligibility purposes.

5. I acknowledge that scholarships are awarded per child and per week.
Any changes to my family’s registration may affect the final scholarship amount.
