1.) Voluntary Participation
I am voluntarily choosing to receve a permanent jewelry service involving the use of a micro-welding device to attach a chain directly around my wrist, ankle, neck, or stomach without a clasp.
2.) Risk Acknowledgement
I understand that there are inherent risks associated with the use of heat and welding tools, including minor burns, skin irritation, or allergic reaction. I accept full responsibility for these risks.
3.) Medical Conditions & Allergies
I confirm that I have disclosed any allergies to metals (such as gold or silver), skin sensitivities, or medical conditions.
4.) Aftercare Responsibility
I understand that aftercare is my responsibility and that I will receive instructions on how to properly care for my jewelry.
5.) Release of Liability
I release The Infinite Link, it's owner(s), employees, and agents from any and all liability related to this service.
6.) Jewelry Policy
I understand that jewelry is intended to long-lasting but not indestructible. The Infinite Link is not responsible for lost or broken jewelry after the appointment. Refunds or exchanges will not be provided for broken or lost jewelry.
7.) Consent to Procedure
I am 18 years or older OR have a legal guardian's signature.
8.) Photo Release
I give permission to The Infinite Link to use my image.
