Between Thinking Caps for Kids Ltd T/A KidsLink ("the Company") And ("the Customer"):
By completing this form, you agree to become a service for schools and families to find on the KidsLink website for help with learning, behaviour, educational, physical and emotional needs.
KidsLink will create an information page for your service explaining who you are, what you do and how you can help kids and teens.

Help for Kids Programme - Make a contribution
The KidsLink "Help for Kids" programme is available to support children and teenagers who attend KidsLink affiliated schools access KidsLink services.

Contributionfs to our Help for Kids fund are invoiced separately and are collected in a separate bank account for distribution to our KidsLink schools. Who apply for support for students from their school.
$50 contribution - your will be sent a separate invoice.*

All products and services your purchase from us are subject to our Terms of Trade.

Our Terms of Trade exclude certain typres of liability and limit our total liability to you in the event of any problems.

The services you have requested will be used primarily for business purposes.

You are, or have the authority to contract on behalf of, the person listed above.

You agree to pay the invoices as required in this agreement.

You agree to pay all and any invoices relating to the 12 months term.

I solomnly and sincerely declare that the Customer:
1. Complies with the required standards and professionalism set by the industry and/or regulatory authority and/or relevant legislation in which the customer practices.
2. Can verify any qualifications, experience claimed, and/or testimonials referred to.
3. Confirms that any person who may come into contact with children or young people has no criminal history that would, in the opinion of the general public, make that person unsuitable ot work with children or young people and have been police vetted.
4. shall at all times exercise responsible care to do good and reduce the risk of harm to others.
5. I am authorised by the Customer to make this declaration.
