By submitting this form, I acknowledge and agree to the following:

I voluntarily choose to participate (or allow my child to participate) in open gyms, tournaments, or other volleyball activities organized by Reno Grass Volleyball. I understand that volleyball involves physical exertion and inherent risks, including but not limited to injury from falls, collisions, uneven surfaces, or heat-related illness. I assume full responsibility for these risks.

I release and hold harmless Reno Grass Volleyball, its organizers, volunteers, staff, and affiliates from any and all liability for injuries, medical costs, or damages incurred during or in connection with any event.

I certify that I (or my child) am physically able to participate and give permission for staff to seek emergency medical care if necessary. I accept financial responsibility for any related costs.

If my child is participating without a parent or guardian present, I understand they will be under the temporary supervision of Reno Grass Volleyball staff or volunteers who have passed background checks and completed SafeSport and concussion training. I give permission for this arrangement.
Media Release

I give Reno Grass Volleyball permission to use photographs, videos, or audio of me (or my child) taken during events for promotional purposes. I understand these may be used in print, on social media, or the organization’s website. I understand I may contact Reno Grass Volleyball in writing to opt out at any time.
